You can choose from any of the forms below, complete it and submit it by logging onto Citibank Online.

Easy Steps to submit your forms to us:
Step 1

Select and print any of the documents from the above 15 documents.

Customers can also choose to write down all the necessary details/instructions of a document, on a piece of paper and send us the handwritten document.

Step 2

Scan or take a picture of the document.

Customers can choose to take a picture with your phone camera should a scanner be unavailable.

Step 3

STEP 3

Log on to Citibank Online, select SERVICES > DOCUMENT UPLOAD and submit your document to us.
